Key Duties and Responsibilities :

Main Objectives:

· To provide high quality support to people using our service in line with the organisations philosophy and principles.

· To participate in the support planning process, ensuring that people receive the type and amount of assistance they need to live in safety and dignity.

· To work closely with other team members to ensure a co-ordinated approach is adopted in relation to how services are delivered.

Responsibilities:

· Embrace the way forward for self-directed teams

· To organise focus persons holidays

· To support focus person maintain contact with family and friends

· Support focus person with daily living, treating each individual with respect.

· To support focus person maintain and enhance their choices through life

· To empower focus people to fulfil their potential

· To support focus people with community living

· To support focus person with all financial transactions.

· To update focus persons support plan

· To ensure focus persons outcomes are met and maintained/develop new outcomes

· To support and coach new staff/team members

· To work as part of a team

· To be able to work on your own initiative

· To be flexible

· Ensure all paperwork is completed and up to date/order paperwork when required

· Take responsibility of the duty rota – cover annual leave and sickness

· Take responsibility for own training using matrix system/attend training when nominated.

· To take responsibility for attending team meetings

· To take responsibility to chair and minute team meetings

· To prepare and ensure availability for supervision sessions

· To ensure all mail received is recorded and dealt with

· To make and attend all health appointments.

· To administer/order medication/to complete daily medication stock checks.

· To perform/assist with personal care

· Adhere to all dietary needs

· To liaise and work along with other agencies

· To promote independence

· To ensure focus persons house is a safe and healthy environment

· To ensure all smoke/door alarms are tested and checks completed

· To record log book ensuring this is accurate and legible

· To ensure log book, communication book and diary are read at start of shift.

· To follow all guidelines relating to focus person

· Adhere to company policies and procedures, uphold government legislation

· Ensure confidentiality is adhered to at all times

· To maintain your registration with SSSC (Scottish Social Services Council)


Essential Criteria

Be able to drive, Varied shift patterns and hours of contracts – must be able to work weekends
